During your initial visit, please bring your insurance information. This will help us expedite your reimbursement after your treatment is complete.
We are happy to bill most insurance companies if we are provided with the correct paperwork. We must also have an updated insurance card with your correct information. If any changes are made to your insurance account or policy, please inform us immediately.
Please keep in mind that your insurance policy is between you and your insurance provider; we are not a part of your contract. It is your responsibility to know your benefits and talk with your insurance company if treatments are not paid for, or if they covered less than you anticipated. If treatment fees are not paid within 45 days, they are considered past due, and you are responsible for paying those fees in full.
Insurance benefits do not guarantee that your insurance provider will cover treatment costs; it is an estimate. Your surgical deposit is an estimate of what you will have to pay, based on the information we receive about your insurance plan. If your insurance company does not pay, you are responsible for all payments and will receive a bill from our office.
Please keep in mind that you are responsible for all charges from Aspire Surgical, regardless of whether your insurance covers them.
After your treatment, we will send you a monthly bill. Most insurance companies take four to six weeks to respond to a claim. If your bills from our office do not reflect payments made by your insurance, please contact our office. Once your insurance has paid, you are responsible for any remaining balance. We appreciate your prompt remittance.
Depending on your situation, our doctors and team can establish a payment plays that fits your needs; however, this plan must be in place prior to the completion of any procedures.
All surgical fees are due before services are rendered. This includes co-payments, deductibles, and non-covered services, which are services that are not paid for by your insurance. These payments are required in full at the time of your treatment, or upon notice of a denied insurance claim.
